To make a claim, simply follow these easy steps.

For seamless cash claims reimbursement, please follow the steps here, in addition to the following cash claims requirements:
Medical report from the doctor, signed and stamped, detailing the medical complaint, procedures performed, and final diagnosis.
Original invoices for doctor’s fees (receipt vouchers), signed and stamped.
Prescription for medication written by the doctor, signed and stamped.
Doctor’s order for X-ray imaging or lab tests, signed and stamped.
Original pharmacy invoices, stamped.
Detailed lab test results or X-ray invoices with the corresponding result report, signed and stamped.
For ER visits, in addition to the documents listed above, a detailed invoice showing all materials and medications used, signed and stamped.
Copy of the insurance card.
IBAN number for the employee (HOF).
